With Studio Manager's Templates feature, it is easy to create custom templates to streamline your workflow. Templates can be applied to future documents, so they are a great way to get a head-start. In this article, we will go over how to create, design, and apply a questionnaire template.

Creating a Questionnaire Template
There are two ways that you can create a questionnaire template: from scratch within the Templates page, or from an existing questionnaire.
Creating a Questionnaire Template from the Templates page
To create a brand new questionnaire template from scratch, go to Studio Manager Dashboard > Templates. Then, click New Template > Questionnaire.
From there, you can create a blank questionnaire template or work off of one of our available sample templates.
Creating a Questionnaire Template from an existing Questionnaire
If you have an existing questionnaire that you want to use as a template, you can also create a template based off of it!
To do this, click into the specific questionnaire from your Questionnaires page. Then, in the top right hand corner, click Actions > Create template.
You will be taken to a new questionnaire template based off of the specific questionnaire, which you can edit further as needed!
Designing a Questionnaire Template
Studio Manager provides intuitive tools to help you easily build out your questionnaire template.
Title and Description Options
To customize the title of your questionnaire template, simply click directly onto the title in the top-left hand corner and edit it.
Under your questionnaire's title, you will have an optional description box that you can fill out. This is a useful area that you can use to greet your client or outline the purpose of the questionnaire.
Adding Questions and Content
It is easy to add different kinds of questions and content blocks to your questionnaire template:
Multiple Choice
Multiple choice questions can be used to list other several possible answers, from which the client can choose their preferred choice.
To add more specific options, simply click Add Option at the bottom. If you want to provide a space for your client to specify their own option, you can click Add "Other" to automatically include an "Other" option where they can fill in their own response.
Short Text
The short text block allows you to accept short, freeform responses from your client.
If you'd like, you can add optional placeholder text in the response field to give your client an idea of how to respond.
Long Text
The long text block allows you to accept longer, freeform responses from your client.
Similar to the short text block, you can add optional placeholder text in the response field to give your client an idea of how to respond.
Checkboxes
The checkbox question type lets your client select more than one answer from a list of choices you provide.
To add more specific options, simply click Add Option at the bottom. If you want to provide a space for your client to specify their own option, you can click Add "Other" to automatically include an "Other" option where they can fill in their own response.
The email block allows you to collect an email address from your client.
Date
The date block allows your client to select a calendar date (e.g. 01/15/2022), as well as a time (e.g. 11:30 AM) if enabled. This is a great block to use if you'd like your client to indicate a specific date for an upcoming event.
To allow your client to select a specific time alongside the date, simply click Add "Time" at the bottom.
Explanation
The explanation block allows you to share additional information with your client. This is a great way to provide more detailed instructions to your client before they continue to a specific section of the questionnaire.
The formatting tools provided above the content area allows you to customize your message as you'd like.
Managing Questions and Content
After you have added your questions and content, you can easily edit them again if you need to make any changes. There are a few other options in the toolbar that will help you manage and customize the questionnaire template, which you can find by hovering over each question.
- Set as Required - Each question will have a required toggle which will allow you to determine if the question needs to be answered by your client or is optional.
- Move - Use the up or down arrows to change the order of the question in your questionnaire.
- Delete - Remove the question from the questionnaire.
Template Settings
In the left-hand panel, you can toggle on different settings that will automatically be applied to any questionnaires you create using this template.
These premium template settings are available to users on an upgraded Studio Manager Plan or any Suite Plan, and are as follows:
- Document Expiry: Set questionnaire to automatically cancel if it is not completed by a certain number of days after being sent (e.g. 3 days after, 7 days after, 14 days after, etc.). Canceled questionnaires cannot be completed.
- Document Reminders: Automatically send email reminders to clients who have not yet signed the questionnaire after a certain number of days (e.g. 3 days after, 7 days after, 14 days after, etc.).
Additional Design Options
To edit the overall design of your questionnaire template, click Edit Design in the upper right hand corner. From within the Edit Design panel, you can select a specific Cover Image for this template.
If you'd like to access additional design settings for all your documents, such as assigning a default Cover Image, Color Theme, and Font Theme, go to your Studio Manager Dashboard > Settings > Branding.

Additional Actions
While editing your questionnaire template, you can take the following actions by selecting Actions in the top right hand side. The quick actions for a questionnaire template include: Edit name, use this template, duplicate, and delete.
- Edit name: Edit your questionnaire's name.
- Use this template: Apply this template to a questionnaire for a specific client.
- Duplicate: Create a copy of your current questionnaire as a separate questionnaire template.
- Delete: Remove the questionnaire template from your account. This action is permanent and cannot be undone.
Applying a Questionnaire Template to a new Questionnaire
Once you have created your questionnaire template, you can easily apply it to new questionnaire!
If you are on your Questionnaires page, you can click New Questionnaire in the top right hand corner. After you select a client for your questionnaire, you will be asked if you'd like to start with a blank questionnaire or one of your existing templates.
If you are starting from your Templates page instead, you can click into your template and select Actions > Use this template.
You will be asked to select a client to create a new questionnaire for, using your template.
